Follow ups
If you need a follow up appointment about an issue that you have already spoken to a member of our clinical team about in the last 6 weeks, then please call the Practice.
Telephone: 0118 966 9333 - We offer a “call back” facility.

You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Sickness certificates (fit notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.
